COVID-19 vaccination providers participating in the CDC COVID-19 Vaccination Program are required to sign a CDC COVID-19 Vaccination Program Provider Agreement. Providers are responsible for adhering to all requirements outlined in the agreement. The CDC COVID-19 Vaccination Program provider agreements include footnoted weblinks for recommendations, requirements and other guidance that are incorporated into the requirements of the vaccination program. In addition, other useful information for providers in the program can be accessed through those websites and this webpage. Providers and organizations enrolled in the program must monitor and comply with any updates to CDC COVID-19 Vaccination Program requirements listed on these websites.
